Para experimentarlas de la forma m\u00e1s aut\u00e9ntica posible, es esencial que los usuarios de un implante coclear (IC) puedan disfrutar de una experiencia auditiva lo m\u00e1s natural posible. Adem\u00e1s, esta experiencia auditiva casi natural ayuda al cerebro a comprender y procesar mejor los sonidos, incluso los complejos. Esto facilita a los usuarios de IC seguir conversaciones en grupos grandes o en entornos ruidosos [1] [2].<\/p>\n

 <\/p>\n

\u00bfC\u00f3mo funciona una audici\u00f3n lo m\u00e1s natural posible con un implante coclear?<\/strong><\/h2>\n

En la audici\u00f3n natural, los sonidos viajan en forma de ondas sonoras desde el o\u00eddo externo a los huesecillos del o\u00eddo medio y de ah\u00ed al o\u00eddo interno. Aqu\u00ed se encuentra la c\u00f3clea con sus miles de diminutas c\u00e9lulas ciliadas, cada una de las cuales responde a una frecuencia sonora espec\u00edfica. Si las c\u00e9lulas ciliadas no son capaces de transmitir la informaci\u00f3n sonora (p\u00e9rdida auditiva neurosensorial), un sistema de implante coclear puede ayudar. Consiste en un procesador de audio que se lleva en la parte exterior de la oreja y un implante que se inserta quir\u00fargicamente bajo la piel detr\u00e1s de la oreja. El implante consta de un encapsulado con una bobina y un im\u00e1n, as\u00ed como un electrodo que se inserta en la c\u00f3clea. Precisamente este electrodo es un factor decisivo para la experiencia auditiva del usuario. Cuanto m\u00e1s precisa sea su colocaci\u00f3n, con mayor exactitud podr\u00e1 activar con un impulso el\u00e9ctrico la estructura nerviosa responsable de la frecuencia sonora correspondiente.<\/p>\n

La longitud del electrodo es decisiva<\/strong><\/h2>\n

Para reproducir los sonidos de la forma m\u00e1s natural y precisa posible, el electrodo del implante coclear debe ser lo suficientemente largo para estimular la c\u00f3clea en toda su longitud, hasta una profundidad de unas dos vueltas (720\u00b0). Es la \u00fanica manera de cubrir todo el espectro sonoro. Esto se debe a que en la c\u00f3clea cada frecuencia se codifica en un lugar determinado: las frecuencias altas se asignan a <240\u00b0, las medias a <480\u00b0 y las bajas a <720\u00b0. Los implantes cocleares con gu\u00edas de electrodos m\u00e1s cortas tambi\u00e9n pueden estimular los tonos bajos, pero s\u00f3lo pueden hacerlo en las zonas de la c\u00f3clea a las que llegan [1] [3] [4]. Como resultado, las frecuencias bajas se alteran hacia arriba, los sonidos parecen diminutos y finos, y el sonido pierde plenitud [5] [6] [7] [8] [9]. En el lugar adecuado en el momento adecuado<\/strong><\/h2>\n

Un electrodo m\u00e1s largo no garantiza por s\u00ed solo la sensaci\u00f3n auditiva m\u00e1s natural posible. Esto se debe a que en la segunda vuelta de la c\u00f3clea hay una particularidad decisiva que tambi\u00e9n deben tener en cuenta los implantes cocleares: entra en juego otro tipo de codificaci\u00f3n del sonido. Adem\u00e1s de la codificaci\u00f3n espacial, la segunda vuelta utiliza la codificaci\u00f3n temporal o codificaci\u00f3n por tasa de estimulaci\u00f3n. \u00a0La codificaci\u00f3n espacial (tonot\u00f3pica) del sonido determina qu\u00e9 c\u00e9lulas ciliadas se estimulan. La codificaci\u00f3n de la tasa temporal es responsable de la rapidez o lentitud con que las c\u00e9lulas ciliadas se activan o desactivan de nuevo. Si el implante no tiene en cuenta esta codificaci\u00f3n temporal, se produce una “confusi\u00f3n de tono”, ya que el cerebro entender\u00eda una tasa r\u00edgida como un desplazamiento hacia arriba del tono [4] [10] [11].
